I have been working on a get home bag. Let's say some sort of disaster happens that leaves streets and roads gridlocked between home and work, or there might be people on the roads you don't want to meet. I am thinking specifically of rioters and looters. If you need to get to home and family, you have to walk and stay out of sight.

Mine has the following items so far:

Compass - keeps you from walking in circles.

Two ways to start a fire - you may need a fire

Water purification tablets - you WILL need water if you have to walk any distance and some way to purify water is lighter than trying to carry what you will need. You could start with a carry bottle and gather it as you go.

Paracord - 1001 uses, or if you have a poncho or a tarp you could rig a shelter if you had to spend the night or wait out a storm.

    Fixed blade knife

    Signal mirror

    Swiss Army knife

    Flashlight and batteries

First aid kit (you can buy one or make one yourself) You need band aids and the usual stuff, plus whatever medications you take regularly in case you get stranded.

Small transistor radio (it would be nice to know what's going on)

Cell phone and a way to charge it, such as a solar cell phone charger.

Non-perishable snacks (some people recommend things like canned sardines and energy bars - I don't know which one I hate more but they will both keep you going)

Since I work in the country and live in town I would have to walk cross country if I wanted to use the shortest distance between the two points, so I should probably have good walking shoes handy (broken in, of course). I don't know just how handy the signal mirror would be but it strikes me that if you needed it, you'd need it, and they are cheap and light. Say you break your leg and you need to signal someone who might be looking for you.

The rest of the stuff is self-explanatory.

If legal to carry, you might pack some ammo or maybe a small .22 and some ammo to save weight. You may also consider a small folding cart with bungee straps to help carry your stuff.

better include a pistol, soft armor, night vision goggles and have enough sense to not try it during daylight hours.
