Perhaps ballot request forms were sent out to all registered voters as a new policy to encourage mail in voting. Those are not actual ballots so nothing out of the ordinary. I imagine before you could pick one up off a stack at the post office.
You yourself said your grandmother voted in 2016 and 2018, either one makes her registered now. They are not searching obit pages for deceased voters, they purge by either inactivity or returned undeliverable mail. By sending applications they actually will be able to update the rolls.
If you opened your grandmother's mail or failed to mark it return to sender or deceased you have violated postal laws and I expect you to March down to the post office and turn yourself in.
There are ballots being sent only to those that request them. Ballot harvesting as you suggest is near impossible to get away with because the harvesters would have to intercept the mail for each one. Not likely. Also, if you don't get your requested ballot you should inform the authorities so they can send you a new one and invalidate the old. Plus the harvesters have to forge all those signatures.
Next up is going door to door to collect ballots offering to witness them and then mail them. Each and every one of those is a witness against you and it takes only one to call the cops. If you're found with other people's ballots you go to prison.
As to those counting the ballots. There are easy ways to spot ballot stuffing and that has always been the case. Remember paper ballots? That's exactly what a mail in ballot is. Those checks have been in place for decades, there's no reason to expect them to fail now.
Using your analogy, there will fewer attempts let alone successes of voter fraud than deaths from covid-19 between now and election day.

Recently, in part of a mass mailing of absentee ballot applications in Michigan on May 19, it was found that applications were being sent to dead people, the Detroit News reported. But experts said the mailings aren’t a cause for concern and can actually help the state update its voting rolls.

Tracy Wimmer, a spokeswoman for Michigan Secretary of State Jocelyn Benson, told the Detroit News that forging a signature is a crime and is “rarely attempted” and that people can mark the ballot as “moved” or “deceased.” By doing so, the state is able to improve its voter list.

The National Conference of State Legislature's site says, "When the ballot is returned to the election office, election officials have a process for examining each and every signature and comparing it to other documents in their files that contains the voter signature – usually the voter registration record."

The signature match is done by election officials who are sometimes assisted by technology and working in bipartisan teams during the process. In some states, individuals go through training to analyze potential voting fraud. If a discrepancy is found, voters are offered an opportunity to provide more verification before election officials decide not to count the ballot.

A recent case study by Stanford Law School on California's Every Vote Counts Act found that many counties there have been hesitant to shift toward automatic signature scanners due to concerns over accuracy. When a signature does not match, a voter is contacted via mail and often as well by phone and email; some counties use SMS messaging or Facebook to notify voters an additional time.

Resealed inside the appropriate envelopes does not mean that they were "These were naked ballots. They were sent WITHOUT the secrecy envelope...which made them invalid and likely WHY they were discarded"
