DVT can and is usually caused by the needle passing substance into a vein and not the intended muscle, regardless of what the substance is! I think it is usually advisable to use aspiration during injections, especially when there is such an intense number being administered, but is is simply not practical to do so The chances (although very, very low) of hitting a vein is inevitable in such extreme numbers.

The idea that they are all afraid to be honest is in the realms of the crazy batshit world of the conspiracy theorist, who thinks casts of thousands of conspirators can all keep quiet.
