There are no other venues to create residencies, since residency cost money, there is no private organization that is interested in organizing a residency program.
Even if some private entity decides to so, most likely their effort will be rejected by Private Medical cartel.
The government is bad, but delegation of public health care to private organization is just an outright corruption.

It's not obligated to fund all of your needs.
Departments are free to fund their own residents.
They get lots of cheap labor from them.
