One way to survive is that parents would have 10 kids to have 5 or 6 babies survive to adulthood.

Its fascinating to see grave yards of families in the late 1800s and early 1900s. The double impact of antibiotics and vaccines is what produced some of those very steep up-rising curves of the population increase in the middle 1900s.

I firmly believe that vaccines are generally far more dangerous than the diseases they are intended to prevent. This certainly applies in the case of Tetanus. What's even more astonishing is that there is a lot of evidence that I have found no evidence that tetanus vaccinations work at all and some evidence that they really don't. From an article from Dr. Kris Gaublomme:

In 1968, the National Communicable Disease Center, USA, mentions a case of tetanus in a fully immunised person (11). Antibodies were below the ‘protective’ level in a patient after three doses of DPT, as discovered by Peebles (7).

Goulon (1972) saw tetanus occurring in 10 out of 64 “immunised” patients (12), Berger (1978) made observations about tetanus in well-vaccinated patients (13). Passen & Andersen (1986) relate a case in a 35-year-old man who developed tetanus despite the fact that he was found to have a neutralising antibody level 16 times that considered protective (5). He was fully vaccinated in childhood and had had boosters up to 4 years before the accident.

The 1986 finding strongly suggest that Dr. Obukhanych's suggestion that antibodies are not what actually makes someone immune to a disease was correct- that, instead, it is the agglutinins that are the important part- and that vaccines don't get the body to produce those.

From what I've read, vaccines and even antibiotics are not the primary reason that tetanus has gone down. Instead, it is hygiene. Here's a good excerpt from Magda Taylor, from the Informed Parents Newsletter UK:
    **
At the beginning of the 1900s the annual death-rate was 7 per million population compared to, for example, whooping cough death-rate, which was 1000 per million children, so even then it was relatively rare considering the living conditions and the deposits by horse-drawn transport still in use.

Tetanus neonatorum is the most common form of tetanus in developing countries. This form of generalised tetanus occurring in the new-born infants is often as a result of an infected umbilical cord stump due to poor hygiene and the use of dirty, rusty scissors when cutting the cord. In the United States, the occurrence of neonatal tetanus was falling before the use of tetanus toxoid became widespread in women, due to improved birth practice.
