As to the Knights, they appear to be some sort of influential order within the RC Church.

There is some sort of link to the island of Malta (near Italy), but I think it has more to do with the fact that they may have ruled over that island, before being driven out.

The Knights of Malta are the descendants of the Knights Hospitaller, one of the two militant monk orders founded during the Crusades (the other being the Knights Templar). The Hospitallers cared for and protected pilgrims to the holy land; the development of the Red Cross comes from the Hospitaller colors of a red cross on a white background and the word "hospital" comes from the same source. They were also quite savage in raiding Muslim areas and interests when not held back by treaty.

After the Holy Land was lost, they moved to Cyprus, where they basically acted as religious pirates on Muslim shipping and ports. Eventually a Muslim force chased them out of Cyprus and they ended up in Malta, where they changed their name and invested heavily in fortifications and continued to attack Muslim ships and coastlines.
In the 1500s a huge Muslim army and fleet besieged Malta and tried to wipe them out, but ultimately failed. This actually repaired some of the diminishing reputation of the Knights of Malta and to a certain extent they morphed into a Catholic society as well as being an actual order.

I seem to remember that the Knights of Malta headquarters is located on Vatican territory in Rome and just as the Vatican is extra-territorial with respect to the nation of Italy, the Knights of Malta are extra-territorial with respect to the Vatican.
